Forms & Strengths
- Oral Tablets: 125 mg, 250 mg
- Oral Solution / Suspension: 125 mg / 5 mL
Adult Dosing
- Mild to moderate infections: 125 mg to 250 mg orally every 6 hours
- Severe infections: 500 mg orally every 6 hours
- Administer on an empty stomach (1 hour before or 2 hours after meals) for optimal absorption
Pediatric Dosing
- Children 1 month to 12 years: 12.5 mg/kg to 25 mg/kg orally every 6 hours
- Maximum daily dose should not exceed adult dosing limits
Indications
- Treatment of mild to moderate infections caused by penicillin-susceptible gram-positive organisms
- Streptococcal pharyngitis and tonsillitis
- Mild skin and soft tissue infections
Mechanism of Action
- Narrow-spectrum, acid-stable natural penicillin derivative
- Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs)
- Leads to lysis and cell death of susceptible bacteria
Contraindications
- Known hypersensitivity to pheneticillin, penicillin, or any beta-lactam antibiotic
- History of penicillin-associated cholestatic jaundice or hepatic dysfunction
Adverse Reactions
- Gastrointestinal: nausea, vomiting, diarrhea, epigastric distress
- Hypersensitivity: rash, urticaria, anaphylaxis, drug fever
- Hematologic: eosinophilia, hemolytic anemia, leukopenia (rare)
- Hepatic: transient elevation of transaminases
Drug Interactions
- Probenecid decreases the renal tubular secretion of penicillins, leading to increased and prolonged serum concentrations
- Bacteriostatic antibiotics (e.g., erythromycin, tetracyclines) may antagonize the bactericidal effect of penicillins
- May reduce the efficacy of oral contraceptives through enterohepatic circulation disruption
