Forms & Strengths
- Injectable powder for reconstitution: 1 g
- Injectable powder for reconstitution: 2 g
Adult Dosing
- General infections: 1 g to 2 g intravenously or intramuscularly once every 24 hours
- Surgical prophylaxis: 1 g intravenously 30 to 60 minutes prior to surgery
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- Lower respiratory tract infections caused by susceptible pathogens
- Urinary tract infections caused by susceptible pathogens
- Skin and skin structure infections
- Septicemia
- Bone and joint infections
- Perioperative prophylaxis
Mechanism of Action
- Second-generation cephalosporin beta-lactam antibiotic
- Inhibits bacterial cell wall synthesis by binding to one or more penicillin-binding proteins (PBPs)
- Possesses an extended half-life allowing once-daily dosing
Contraindications
- Known hypersensitivity to cefonicid, cephalosporins, or other beta-lactam antibiotics
Adverse Reactions
- Injection site reactions (pain, phlebitis)
- Diarrhea
- Nausea and vomiting
- Transient elevations in liver transaminases and alkaline phosphatase
- Hypersensitivity reactions (rash, pruritus, fever, anaphylaxis)
- Hematologic abnormalities (eosinophilia, positive Coombs test, transient leukopenia)
Drug Interactions
- Probenecid decreases renal tubular secretion of cefonicid, leading to increased and prolonged plasma concentrations
- Concomitant use with nephrotoxic drugs (e.g., aminoglycosides, potent diuretics) may increase the risk of nephrotoxicity
