Forms & Strengths
- Injectable powder for reconstitution: 1 g
- Injectable powder for reconstitution: 2 g
Adult Dosing
- Standard dosage: 2 g to 4 g per day divided in equally spaced doses every 12 hours
- Severe infections: Up to 8 g to 12 g per day divided in equally spaced doses every 8 to 12 hours
Pediatric Dosing
- Neonates, infants, and children: 50 mg/kg to 200 mg/kg per day divided every 8 to 12 hours
- Maximum daily dose in pediatrics generally not to exceed 12 g
Indications
- Respiratory tract infections caused by susceptible organisms
- Peritonitis and other intra-abdominal infections
- Bacterial septicemia
- Skin and skin structure infections
- Pelvic inflammatory disease, endometritis, and other female genital tract infections
Mechanism of Action
- Third-generation cephalosporin antibiotic
- Inhibits bacterial cell wall synthesis by binding to one or more penicillin-binding proteins (PBPs)
- Bactericidal activity against a wide range of gram-negative and gram-positive aerobic and anaerobic bacteria
Contraindications
- Known hypersensitivity to cefoperazone, other cephalosporins, or beta-lactam antibiotics
- History of severe hypersensitivity reactions to penicillin
Adverse Reactions
- Diarrhea and pseudomembranous colitis
- Hypoprothrombinemia and bleeding disorders due to N-methylthiotetrazole side chain
- Disulfiram-like reaction when ingested with ethanol
- Transient elevation of liver function tests (AST, ALT, alkaline phosphatase)
- Eosinophilia, thrombocytopenia, and positive direct Coombs test
- Injection site pain and phlebitis
Drug Interactions
- Ethanol: causes disulfiram-like reaction (flushing, sweating, headache, tachycardia) up to 72 hours post-administration
- Anticoagulants (warfarin, heparin): increases risk of bleeding due to additive hypoprothrombinemia and platelet inhibition
- Aminoglycosides: potential for synergistic bactericidal activity, but must be administered at separate sites to avoid inactivation
