ceftobiprole medocaril
J01DI01
Forms & Strengths
- Powder for solution for infusion: 666 mg ceftobiprole medocaril sodium (equivalent to 500 mg ceftobiprole) per vial
Adult Dosing
- Staphylococcus aureus bacteremia (SAB): 500 mg intravenously every 8 hours administered over 2 hours
- Acute bacterial skin and skin structure infections (ABSSSI): 500 mg intravenously every 8 hours administered over 2 hours
- Community-acquired bacterial pneumonia (CABP): 500 mg intravenously every 8 hours administered over 2 hours
- Hospital-acquired and ventilator-associated pneumonia (HAP/VAP): 500 mg intravenously every 8 hours administered over 2 hours
- Renal dosage adjustment (CrCl 30-50 mL/min): 400 mg every 8 hours
- Renal dosage adjustment (CrCl 15-29 mL/min): 300 mg every 8 hours
- Renal dosage adjustment (CrCl <15 mL/min including ESRD/HD): 200 mg every 8 hours
Pediatric Dosing
- Safety and effectiveness in pediatric patients below 18 years of age have not been established
Indications
- Staphylococcus aureus bacteremia (SAB) including right-sided infective endocarditis
- Acute bacterial skin and skin structure infections (ABSSSI)
- Community-acquired bacterial pneumonia (CABP)
- Hospital-acquired bacterial pneumonia (HABP) and ventilator-associated bacterial pneumonia (VABP)
Mechanism of Action
- Advanced-generation, broad-spectrum, bactericidal cephalosporin
- Exerts antibacterial activity by binding to essential penicillin-binding proteins (PBPs), including PBP2a in methicillin-resistant Staphylococcus aureus (MRSA) and PBP2x in penicillin-resistant Streptococcus pneumoniae
- Inhibits bacterial cell wall synthesis leading to cell lysis and death
Contraindications
- Known severe hypersensitivity to ceftobiprole or other cephalosporin-class antibacterial drugs
- History of severe hypersensitivity (e.g., anaphylactic reactions) to other beta-lactam antibacterial drugs
Adverse Reactions
- Nausea
- Vomiting
- Diarrhea
- Hypokalemia
- Insomnia
- Hypertension
- Elevated hepatic transaminases
- Rash
- Anemia
- Clostridioides difficile-associated diarrhea
Drug Interactions
- No significant hepatic cytochrome P450-mediated drug-drug interactions expected
- Concomitant administration with nephrotoxic drugs (e.g., aminoglycosides, loop diuretics) may increase the risk of renal impairment
- Probenecid may decrease the renal excretion of ceftobiprole
