Forms & Strengths
- Tablets: sulfamerazine 200mg/trimethoprim 40mg, sulfamerazine 400mg/trimethoprim 80mg, sulfamerazine 800mg/trimethoprim 160mg
- Suspension: sulfamerazine 200mg/trimethoprim 40mg per 5ml
Adult Dosing
- Urinary tract infections: 1600mg/320mg (sulfamerazine/trimethoprim) PO q12h for 10-14 days
- Acute otitis media: 1600mg/320mg (sulfamerazine/trimethoprim) PO q12h for 10 days
Pediatric Dosing
- Children >2 months: 8-10mg/kg trimethoprim (40-50mg/kg sulfamerazine) PO q12h for 10 days
- Children <2 months or <3kg: not recommended
Indications
- Urinary tract infections
- Acute otitis media
- Acute bronchitis
- Traveler's diarrhea
Mechanism of Action
- Sulfamerazine inhibits bacterial folic acid synthesis
- Trimethoprim inhibits dihydrofolate reductase, blocking tetrahydrofolic acid synthesis
Contraindications
- Pregnancy at term
- Infancy <2 months
- Severe renal impairment (CrCl <15ml/min)
- Hypersensitivity to sulfonamides or trimethoprim
Adverse Reactions
- Nausea, vomiting, diarrhea
- Rash, pruritus
- Bone marrow suppression, agranulocytosis, thrombocytopenia
- Hepatotoxicity, jaundice
Drug Interactions
- Warfarin: increased anticoagulation
- Methotrexate: increased toxicity
- Phenytoin: increased phenytoin levels
- Cyclosporine: increased cyclosporine levels
