Forms & Strengths
- Tablet: 500 mg
- Granules for oral suspension: 250 mg/5 mL
Adult Dosing
- Standard: 1 g to 2 g daily divided into 2 to 3 doses
- Severe infections: Up to 3 g daily in divided doses
Pediatric Dosing
- Children > 1 month: 30 to 50 mg/kg/day divided into 2 to 3 doses
- Maximum daily dose based on weight and clinical severity
Indications
- Upper and lower respiratory tract infections caused by susceptible organisms
- Skin and soft tissue infections
- Dental and oral infections
- Genitourinary tract infections including Chlamydia trachomatis
Mechanism of Action
- Macrolide antibiotic inhibiting bacterial protein synthesis
- Binds reversibly to the 50S ribosomal subunit, inhibiting transpeptidation and translocation
Contraindications
- Hypersensitivity to josamycin or other macrolide antibiotics
- Severe hepatic impairment
- Concurrent administration with ergot alkaloids
Adverse Reactions
- Gastrointestinal: nausea, vomiting, abdominal pain, diarrhea
- Hepatic: transient elevation of liver enzymes, cholestatic hepatitis
- Hypersensitivity: rash, pruritus, urticaria
Drug Interactions
- May increase serum concentrations of digoxin
- May enhance anticoagulant effect of vitamin K antagonists
- Risk of ergotism with concurrent ergot alkaloids
- Potential prolongation of QT interval with class IA and III antiarrhythmics
