Forms & Strengths
- injection: 10mg/mL, 40mg/mL
- ophthalmic solution: 0.3%
- ophthalmic ointment: 0.3%
- inhalation solution: 300mg/5mL
Adult Dosing
- severe infections: 2-3mg/kg IV q8h
- moderate infections: 3-5mg/kg IV q8h
- cystic fibrosis: 300mg inhalation q12h
- ophthalmic infections: 1-2 drops q4-6h
Pediatric Dosing
- term neonates >1 week: 2mg/kg IV q8h
- preterm neonates: 1.5-2mg/kg IV q8h
- children >1 year: 2.5-3.5mg/kg IV q8h
- cystic fibrosis: 300mg inhalation q12h
Indications
- severe gram-negative infections
- cystic fibrosis with Pseudomonas aeruginosa
- ophthalmic infections
- intra-abdominal infections
- septicemia
Mechanism of Action
- inhibits protein synthesis by binding to 30S ribosomal subunit
- interferes with initiation complex of peptide formation
- exerts bactericidal effects
Contraindications
- hypersensitivity to tobramycin or other aminoglycosides
- myasthenia gravis
- parkinsonism
- pregnancy and lactation
Adverse Reactions
- nephrotoxicity
- ototoxicity
- neuromuscular blockade
- allergic reactions
- respiratory distress
Drug Interactions
- increased nephrotoxicity with other nephrotoxic agents
- increased ototoxicity with other ototoxic agents
- increased neuromuscular blockade with neuromuscular blockers
- reduced efficacy with beta-lactam antibiotics
