Forms & Strengths
- Injectable solution: 50 mg/mL
- Injectable solution: 100 mg/mL
- Intravenous infusion vials: 200 mg
- Intravenous infusion vials: 400 mg
Adult Dosing
- Complicated urinary tract infections: 8-15 mg/kg/day divided every 12 to 24 hours IV/IM
- Serious systemic bacterial infections: 15 mg/kg/day divided every 12 to 24 hours IV/IM
- Adjust dose based on renal function and therapeutic drug monitoring
Pediatric Dosing
- Safety and efficacy in pediatric patients not well established
- Use generally restricted to serious infections where safer alternatives are ineffective: 7.5 to 15 mg/kg/day divided every 8 to 12 hours IV/IM
Indications
- Treatment of serious bacterial infections caused by susceptible Gram-negative bacilli
- Complicated and recurrent urinary tract infections
- Lower respiratory tract infections
- Septicemia and intra-abdominal infections
Mechanism of Action
- Semi-synthetic aminoglycoside antibiotic
- Binds irreversibly to the 30S ribosomal subunit of susceptible bacteria
- Inhibits protein synthesis and causes misreading of the genetic code
- Exhibits concentration-dependent bactericidal activity
Contraindications
- Hypersensitivity to isepamicin or other aminoglycosides
- History of severe hypersensitivity or toxic reactions to aminoglycosides
Adverse Reactions
- Nephrotoxicity (elevated serum creatinine, blood urea nitrogen)
- Ototoxicity (auditory damage, vestibular toxicity, tinnitus, hearing loss)
- Neurotoxicity and neuromuscular blockade
- Injection site reactions, rash, nausea, and elevated liver enzymes
Drug Interactions
- Increased risk of nephrotoxicity with concurrent potent diuretics (e.g., furosemide)
- Enhanced nephrotoxicity and ototoxicity with other aminoglycosides, vancomycin, or amphotericin B
- Prolonged neuromuscular blockade when co-administered with neuromuscular blocking agents
- Potentiation of nephrotoxic effects when combined with platinum-based chemotherapeutic agents
