Forms & Strengths
- Tablets: 200mg cefixime and 100mg ornidazole
- Tablets: 400mg cefixime and 200mg ornidazole
- Capsules: 200mg cefixime and 100mg ornidazole
- Suspension: 50mg/5mL cefixime and 25mg/5mL ornidazole
Adult Dosing
- Typhoid fever: 200mg cefixime and 100mg ornidazole twice daily for 7-10 days
- Uncomplicated UTI: 200mg cefixime and 100mg ornidazole twice daily for 5-7 days
- Gonorrhea: 400mg cefixime and 200mg ornidazole single dose
- GI infections: 200mg cefixime and 100mg ornidazole twice daily for 5-7 days
Pediatric Dosing
- Above 6 months: 4-6mg/kg/day cefixime and 2-3mg/kg/day ornidazole in divided doses for 5-7 days
- For children above 50kg, follow adult dosing
- For children with typhoid fever: 6-8mg/kg/day cefixime and 3-4mg/kg/day ornidazole in divided doses for 7-10 days
Indications
- Treatment of typhoid fever
- Treatment of uncomplicated urinary tract infections
- Treatment of gonorrhea
- Treatment of gastrointestinal infections
- Treatment of respiratory tract infections
Mechanism of Action
- Cefixime is a third generation cephalosporin antibiotic with bactericidal action
- Ornidazole is an antiprotozoal and antibacterial agent with action against anaerobic bacteria
- Cefixime inhibits bacterial cell wall synthesis by binding to penicillin binding proteins
- Ornidazole is metabolized to form reactive intermediates that interfere with DNA synthesis
Contraindications
- Hypersensitivity to cefixime, other cephalosporins, or ornidazole
- Pregnancy and lactation (use with caution)
- Patients with a history of seizure disorders
Adverse Reactions
- Diarrhea
- Loose stools
- Abdominal pain
- Nausea and vomiting
- Dizziness and headache
- Allergic reactions such as rash and itching
Drug Interactions
- Warfarin: increases anticoagulant effect
- Aminoglycosides: increases nephrotoxicity risk
- Chloramphenicol: increases risk of granulocytopenia
- Theophylline: decreases theophylline clearance
