J01XA03
Forms & Strengths
- Powder for injection: 250 mg per vial
- Powder for injection: 750 mg per vial
Adult Dosing
- Complicated skin and skin structure infections (cSSSI): 10 mg/kg IV every 24 hours for 7 to 14 days
- Hospital-acquired and ventilator-associated bacterial pneumonia (HABP/VABP): 10 mg/kg IV every 24 hours for 7 to 21 days
- Renal adjustment (CrCl 30-50 mL/min): 7.5 mg/kg IV every 24 hours
- Renal adjustment (CrCl 10-29 mL/min): 10 mg/kg IV every 48 hours
- Renal adjustment (CrCl <10 mL/min): 7.5 mg/kg IV every 48 hours
Pediatric Dosing
- Safety and efficacy in pediatric patients (<18 years of age) have not been established
Indications
- Complicated skin and skin structure infections (cSSSI) caused by susceptible Gram-positive bacteria including S. aureus
- Hospital-acquired and ventilator-associated bacterial pneumonia (HABP/VABP) caused by S. aureus (including MRSA)
Mechanism of Action
- Lipoglycopeptide antibacterial agent
- Inhibits bacterial cell wall synthesis by binding to D-alanyl-D-alanine termini of cell wall peptidoglycan precursors
- Perturbs bacterial cell membrane integrity, leading to depolarization and increased permeability
Contraindications
- Known hypersensitivity to telavancin
- Concurrent use of intravenous unfractionated heparin (due to interference with coagulation tests)
- Pregnancy (due to risk of adverse developmental outcomes based on animal data)
Adverse Reactions
- Taste disturbance (foamy urine, dysgeusia)
- Nausea and vomiting
- Diarrhea
- Insomnia
- Renal impairment / increased serum creatinine
- Infusion-related reactions ('red man syndrome', flushing, pruritus)
Drug Interactions
- Drugs that prolong the QT interval (e.g., class IA or III antiarrhythmics, macrolides): risk of additive QT prolongation
- Nephrotoxic drugs (e.g., aminoglycosides, NSAIDs, amphotericin B): increased risk of renal dysfunction
- Anticoagulants / Heparin: telavancin interferes with routine coagulation assays (aPTT, PT/INR, ACT)
Curated Content: Needs Vetting Before Put to Clinical Use
