oritavancin
J01XA05
Forms & Strengths
- Powder for injection: 400 mg single-dose vial
Adult Dosing
- Acute bacterial skin and skin structure infections (ABSSSI): 1200 mg administered as a single intravenous infusion over 3 hours
Pediatric Dosing
- Safety and efficacy in pediatric patients below the age of 18 years have not been established
Indications
- Treatment of acute bacterial skin and skin structure infections (ABSSSI) caused by susceptible isolates of gram-positive bacteria (Staphylococcus aureus including methicillin-susceptible and methicillin-resistant strains, Streptococcus pyogenes, Streptococcus agalactiae, Streptococcus dysgalactiae, Streptococcus anginosus group)
Mechanism of Action
- Lipoglycopeptide antibacterial agent that inhibits cell wall biosynthesis by binding to stem peptides of cell wall precursors and disrupting bacterial membrane integrity
Contraindications
- Use of intravenous unfractionated heparin sodium within 48 hours of oritavancin administration due to potential risk of falsely elevated activated partial thromboplastin time (aPTT) laboratory results
Adverse Reactions
- Nausea
- Headache
- Vomiting
- Diarrhea
- Limb and subcutaneous abscesses
- Hypersensitivity reactions including anaphylaxis
- Infusion-related reactions (flushing, urticaria, pruritus)
Drug Interactions
- Concomitant intravenous unfractionated heparin is contraindicated for 48 hours after oritavancin administration
- Weak inhibitor of CYP2C9 and CYP2C19; potential to increase exposure of narrow therapeutic index drugs metabolized by these enzymes
