lefamulin
J01XX12
Forms & Strengths
- Oral tablets: 600 mg
- Intravenous injection: 150 mg in 15 mL single-dose vial
Adult Dosing
- Community-acquired bacterial pneumonia (CABP): 600 mg orally every 12 hours for 5 days
- CABP: 150 mg IV over 60 minutes every 12 hours for 5 to 7 days
- Hepatic impairment (Severe, Child-Pugh Class C): IV dosing preferred, adjust if necessary
Pediatric Dosing
- Safety and efficacy in pediatric patients (younger than 18 years) have not been established
Indications
- Treatment of community-acquired bacterial pneumonia (CABP) caused by susceptible isolates of Streptococcus pneumoniae, Staphylococcus aureus (methicillin-susceptible isolates), Legionella pneumophila, Mycoplasma pneumoniae, and Chlamydophila pneumoniae
Mechanism of Action
- Pleuromutilin antibacterial agent that inhibits bacterial protein synthesis by binding to the peptidyl transferase center of the 50S bacterial ribosomal subunit, preventing the normal binding of tRNA
Contraindications
- Known hypersensitivity to lefamulin, pleuromutilins, or any component of the formulation
- Concurrent administration with drugs known to prolong the QT interval and CYP3A4 strong inhibitors/inducers (oral formulation)
Adverse Reactions
- Diarrhea
- Nausea
- Injection site reactions
- Vomiting
- Elevated transaminases
- QT interval prolongation
Drug Interactions
- CYP3A4 substrates (lefamulin is a moderate CYP3A substrate/inhibitor)
- QT interval-prolonging drugs (additive risk of arrhythmia)
- Strong CYP3A4 inhibitors and inducers (adjust oral/IV administration accordingly)
