boceprevir
J05AP03
Adult Dosing
- 800 mg orally three times daily (every 7-9 hours) with food
- Used in combination with peginterferon alfa and ribavirin
- Duration and regimen depend on prior treatment response and HCV RNA levels
Pediatric Dosing
- Safety and efficacy not established in pediatric patients (< 18 years)
Indications
- Treatment of chronic hepatitis C genotype 1 infection in combination with peginterferon alfa and ribavirin in adult patients with compensated liver disease
Mechanism of Action
- HCV NS3/4A protease inhibitor that binds covalently to the active serine residue (Ser139) of the NS3 protease, inhibiting viral replication
Contraindications
- Co-administration with drugs that are highly dependent on CYP3A4/5 for clearance and for which elevated plasma concentrations are associated with serious/life-threatening events
- Co-administration with potent CYP3A4/5 inducers (e.g., rifampin, carbamazepine, St. John's wort)
- Pregnancy and when peginterferon alfa and ribavirin are contraindicated
Adverse Reactions
- Fatigue
- Anemia
- Nausea
- Headache
- Dysgeusia
- Chills
- Insomnia
- Neutropenia
Drug Interactions
- Potent CYP3A4/5 inducers decrease boceprevir concentrations
- CYP3A4/5 inhibitors may increase boceprevir concentrations
- Boceprevir inhibits CYP3A4/5, potentially increasing concentrations of co-administered substrates (e.g., statins, sedatives, antiarrhythmics)
- Coadministration with certain HIV protease inhibitors or non-nucleoside reverse transcriptase inhibitors may alter concentrations
