Indications
- Treatment of human smallpox disease caused by variola virus
- Treatment of mpox (monkeypox) virus infections
- Treatment of non-variola orthopoxvirus infections (e.g., cowpox, vaccinia complications)
Mechanism of Action
- Inhibits the orthopoxvirus VP37 envelope protein
- Blocks formation of the p37-wrapped extracellular virion necessary for intracellular and cell-to-cell spread
- Prevents the virus from exiting the infected host cell and egressing to uninfected tissues
Contraindications
- Known hypersensitivity to tecovirimat or any components of the formulation
Adverse Reactions
- Headache
- Nausea
- Abdominal pain
- Vomiting
- Fatigue
- Injection site reactions (with IV formulation)
Drug Interactions
- Repaglinide: Tecovirimat may decrease repaglinide exposure, potentially resulting in loss of glycemic control
- Midazolam: May decrease exposure of CYP3A4 substrates
- Coadministration with strong CYP3A4 inducers (e.g., rifampin) may significantly decrease tecovirimat plasma concentrations
