Forms & Strengths
- Intramuscular suspension in multidose or single-dose vials/syringes
- Combined DTwP-HepB-Hib vaccine: diphtheria toxoid, tetanus toxoid, whole-cell pertussis, hepatitis B surface antigen, Haemophilus b conjugate
Adult Dosing
- Not routinely recommended for individuals aged 7 years and older due to increased reactogenicity with whole-cell pertussis components
Pediatric Dosing
- Primary series: 0.5 mL intramuscularly at 2, 4, and 6 months of age
- Boosters: 0.5 mL intramuscularly at 15-18 months and 4-6 years of age
Indications
- Active immunization for the prevention of diphtheria, tetanus, pertussis, hepatitis B, and Haemophilus influenzae type b infections in infants and young children
Mechanism of Action
- Induces active immunity by stimulating production of antibodies against Bordetella pertussis antigens, diphtheria and tetanus toxoids, hepatitis B surface antigen, and Hib capsular polysaccharide
Contraindications
- Severe allergic reaction (e.g., anaphylaxis) after a previous dose of any component of the vaccine
- Encephalopathy (e.g., coma, decreased level of consciousness, prolonged seizures) within 7 days of a previous dose of a pertussis-containing vaccine not attributable to another identifiable cause
- Progressive neurologic disorder until the neurologic status is clarified and stabilized
Adverse Reactions
- Injection site reactions (pain, redness, swelling)
- Fever
- Irritability
- Drowsiness
- Persistent crying
- Hypotonic-hyporesponsive episode (rare)
Drug Interactions
- Immunosuppressive therapies (may reduce the immune response to the vaccine)
- Other live or inactivated vaccines (can be administered simultaneously at different injection sites)
