Forms & Strengths
- Suspension for injection: 2.5 international units (IU) per 0.5 mL or 1.0 mL vial/syringe
Adult Dosing
- Pre-exposure prophylaxis: 1 mL intramuscularly (IM) on days 0, 7, and 21 or 28
- Post-exposure prophylaxis (PEP) in non-immunized individuals: 1 mL IM on days 0, 3, 7, and 14, plus rabies immune globulin (RIG) on day 0
- Post-exposure prophylaxis in previously immunized individuals: 1 mL IM on days 0 and 3 (no RIG)
Pediatric Dosing
- Pre-exposure prophylaxis: 0.5 mL or 1 mL IM (depending on product) on days 0, 7, and 21 or 28
- Post-exposure prophylaxis: Same schedule as adults (0.5 mL or 1 mL IM on days 0, 3, 7, and 14), plus weight-based RIG on day 0
Indications
- Pre-exposure prophylaxis of rabies virus infection for individuals at high risk
- Post-exposure prophylaxis following potential exposure to a rabid animal (e.g., bite, scratch, mucous membrane contact)
Mechanism of Action
- Induces active immunity by stimulating the production of virus-neutralizing antibodies against rabies virus glycoproteins
Contraindications
- Hypersensitivity to the active substance or any excipient (e.g., neomycin, streptomycin, polymyxin, beta-propiolactone)
- Post-exposure prophylaxis has no absolute contraindications due to the 100% fatality rate of clinical rabies
Adverse Reactions
- Injection site pain, erythema, swelling, and pruritus
- Mild systemic effects: headache, nausea, abdominal pain, myalgia, dizziness, and low-grade fever
- Rarely: immediate hypersensitivity or anaphylactoid reactions, serum sickness-like reactions
Drug Interactions
- Immunosuppressive agents (corticosteroids, antineoplastics, radiation) may interfere with antibody response, potentially reducing vaccine efficacy
- Do not mix rabies vaccine and rabies immune globulin (RIG) in the same syringe or administer in the same anatomical site
