Forms & Strengths
- Injection: 4mg/4ml vial
- Capsules: 0.25mg, 1mg
- IV solution: 1mg/ml, 4mg/4ml
Adult Dosing
- Ovarian cancer: 1.5mg/m2 IV days 1-5, repeat every 21 days
- Small cell lung cancer: 1.5mg/m2 IV days 1-5, repeat every 21 days
- Cervical cancer: 0.75mg/m2 IV days 1-3, repeat every 21 days
Pediatric Dosing
- Recurrent medulloblastoma or primitive neuroectodermal tumors: 1.4-2.0mg/m2 IV days 1-5, repeat every 21-28 days (in combination with etoposide and carboplatin)
- High-grade glioma: 1.0-1.5mg/m2 IV days 1-5, repeat every 21-28 days (in combination with etoposide and temozolomide)
Indications
- Ovarian cancer
- Small cell lung cancer
- Cervical cancer
- Pediatric recurrent medulloblastoma or primitive neuroectodermal tumors
- Pediatric high-grade glioma (in combination therapy)
Mechanism of Action
- Inhibits topoisomerase I enzyme
- Stabilizes the transient breakage of DNA strands
- Prevents DNA replication and transcription
Contraindications
- Severe myelosuppression
- Breastfeeding
- Hypersensitivity to topotecan or its components
Adverse Reactions
- Neutropenia
- Anemia
- Thrombocytopenia
- Nausea
- Vomiting
- Diarrhea
- Fatigue
- Alopecia
Drug Interactions
- Increased risk of myelosuppression with concomitant G-CSF
- Increased risk of bleeding with concomitant anticoagulants
- Reduced clearance with concomitant CYP3A inhibitors
- Inhibition of P-glycoprotein with concomitant P-gp substrates
