alectinib
L01ED03
Adult Dosing
- Non-Small Cell Lung Cancer (NSCLC), ALK-positive: 600 mg orally twice daily with food until disease progression or unacceptable toxicity
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- First-line and subsequent treatment of anaplastic lymphoma kinase (ALK)-positive, metastatic non-small cell lung cancer (NSCLC) detected by an FDA-approved test
Mechanism of Action
- Selective tyrosine kinase inhibitor targeting ALK and RET; inhibits ALK phosphorylation and ALK-mediated downstream signaling proteins, inducing cell death (apoptosis) in tumor cells
Contraindications
- Known severe hypersensitivity to alectinib or any of its excipients
Adverse Reactions
- Fatigue
- Constipation
- Edema (peripheral, generalized)
- Myalgia
- Increased blood bilirubin and transaminases
- Increased creatine phosphokinase (CPK)
- Anemia
- Bradycardia
Drug Interactions
- Strong CYP3A inhibitors (e.g., ketoconazole): May increase alectinib plasma concentrations
- Strong CYP3A inducers (e.g., rifampin): May decrease alectinib plasma concentrations
- P-gp and BCRP substrates: Alectinib may alter their pharmacokinetics
