Forms & Strengths
- Tablets (Cabometyx): 20 mg, 40 mg, 60 mg
- Capsules (Cometriq): 20 mg, 80 mg
Adult Dosing
- Renal Cell Carcinoma: 60 mg orally once daily
- Hepatocellular Carcinoma: 60 mg orally once daily
- Medullary Thyroid Cancer: 140 mg orally once daily (as capsules)
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- Advanced renal cell carcinoma
- Hepatocellular carcinoma previously treated with sorafenib
- Progressive, symptomatic medullary thyroid cancer
Mechanism of Action
- Multi-kinase inhibitor targeting MET, VEGFR-1, VEGFR-2, VEGFR-3, AXL, RET, ROS1, MER, KIT, TRKB, and FLT-3
- Inhibits tumor angiogenesis, metastasis, and drug resistance
Contraindications
- Severe hypersensitivity to cabozantinib or excipients
Adverse Reactions
- Diarrhea, fatigue, nausea, decreased appetite
- Palmar-plantar erythrodysesthesia syndrome (hand-foot syndrome)
- Hypertension, hemorrhage, severe gastrointestinal perforation or fistulas
- Thromboembolic events, wound healing complications, proteinuria
Drug Interactions
- Strong CYP3A4 inhibitors (e.g., ketoconazole) increase cabozantinib exposure; monitor for toxicity
- Strong CYP3A4 inducers (e.g., rifampin) decrease cabozantinib exposure; avoid concurrent use
- Bile acid sequestrants may bind to the drug; avoid co-administration
