toripalimab
L01FF13
Forms & Strengths
- Intravenous infusion: 240 mg/6 mL single-dose vial
Adult Dosing
- Nasopharyngeal carcinoma (NPC): 240 mg IV every 3 weeks in combination with gemcitabine and cisplatin, followed by toripalimab monotherapy
- Nasopharyngeal carcinoma (Monotherapy): 240 mg IV every 3 weeks for recurrent or metastatic disease after platinum-based chemotherapy
- Esophageal squamous cell carcinoma (ESCC): 240 mg IV every 3 weeks in combination with paclitaxel and cisplatin
Pediatric Dosing
- Safety and effectiveness in pediatric patients have not been established
Indications
- First-line treatment of recurrent or metastatic nasopharyngeal carcinoma in combination with cisplatin and gemcitabine
- Monotherapy for recurrent or metastatic nasopharyngeal carcinoma with disease progression on or after platinum-containing chemotherapy
- First-line treatment of unresectable advanced, recurrent, or metastatic esophageal squamous cell carcinoma in combination with paclitaxel and cisplatin
Mechanism of Action
- Programmed death receptor-1 (PD-1) blocking antibody
- Binds to PD-1 receptors on T cells, preventing interaction with PD-L1 and PD-L2 ligands
- Releases PD-1 pathway-mediated inhibition of the immune response, unleashing anti-tumor T-cell immunity
Contraindications
- Severe hypersensitivity to toripalimab or any of its excipients
Adverse Reactions
- Immune-mediated adverse reactions including pneumonitis, colitis, hepatitis, endocrinopathies, and nephritis
- Infusion-related reactions ranging from chills to severe anaphylaxis
- Common non-immune adverse reactions: fatigue, nausea, decreased appetite, constipation, anemia, and musculoskeletal pain
Drug Interactions
- Systemic corticosteroids or immunosuppressants should be avoided before starting toripalimab due to potential interference with pharmacodynamic activity
- Baseline systemic corticosteroids may be used to treat immune-mediated adverse reactions without affecting efficacy once treatment is initiated
