interferon beta-1a
L03AB07
Forms & Strengths
- Prefilled syringes: 22 mcg/0.5 mL, 30 mcg/0.5 mL, 44 mcg/0.5 mL
- Prefilled pens: 22 mcg/0.5 mL, 44 mcg/0.5 mL
- Lyophilized powder for injection: 30 mcg per vial
Adult Dosing
- Relapsing multiple sclerosis: 30 mcg intramuscularly once weekly
- Relapsing multiple sclerosis: 22 mcg or 44 mcg subcutaneously three times weekly
- Titration: Initiate at lower doses (e.g., 8.8 mcg SC 3x/week) and increase over 4 weeks to target dose to reduce flu-like symptoms
Pediatric Dosing
- Safety and efficacy not established in pediatric patients below 12 years of age
- Pediatric patients aged 12 to 17 years: 22 mcg or 44 mcg subcutaneously three times weekly, or 30 mcg intramuscularly once weekly
Indications
- Treatment of relapsing forms of multiple sclerosis (MS) to slow the accumulation of physical disability and decrease frequency of clinical exacerbations
- Clinically isolated syndrome (CIS) in patients with a first clinical episode and MRI features consistent with multiple sclerosis
Mechanism of Action
- Recombinant interferon beta that binds to specific cell surface receptors, initiating a cascade of intracellular events
- Upregulates expression of interferon-stimulated genes, downregulates expression of pro-inflammatory cytokines, and reduces antigen presentation
- Reduces blood-brain barrier permeability and suppresses T-cell proliferation, reducing inflammatory central nervous system activity
Contraindications
- Hypersensitivity to natural or recombinant interferon beta or human albumin
- Severe depressive illness and/or suicidal ideation
Adverse Reactions
- Flu-like syndrome (myalgia, fever, chills, fatigue, headache, nausea)
- Injection site reactions (erythema, pain, necrosis)
- Hepatic injury (elevated transaminases, autoimmune hepatitis, severe liver failure)
- Depression, suicidal ideation, and anxiety
- Leukopenia, lymphopenia, and anemia
- Seizures and thrombotic microangiopathy
Drug Interactions
- Cytochrome P450 inducers/inhibitors: Theoretical risk of altering metabolism, though interferon beta is not known to be a significant CYP substrate or inhibitor
- Myelosuppressive agents: May increase risk of bone marrow suppression and cytopenias when used concomitantly
- Other hepatotoxic medications: Potential for additive hepatotoxicity
Curated Content: Needs Vetting Before Put to Clinical Use
