plerixafor
L03AX16
Forms & Strengths
- Injection: 24 mg/1.2 mL (20 mg/mL) in single-dose vials
Adult Dosing
- 0.24 mg/kg actual body weight subcutaneously approximately 11 hours prior to initiation of apheresis
- Maximum daily dose: 40 mg (based on estimated body weight of 175 kg)
- Administer daily for up to 4 consecutive days
Pediatric Dosing
- Safety and efficacy not established in pediatric patients under 18 years
- Pharmacokinetic data in pediatric patients aged 1 to less than 18 years support a weight-based dose similar to adults (0.24 mg/kg)
Indications
- Mobilization of hematopoietic stem cells to the peripheral blood for collection and subsequent autologous transplantation in patients with non-Hodgkin's lymphoma or multiple myeloma
Mechanism of Action
- Selective and reversible antagonist of the C-X-C chemokine receptor type 4 (CXCR4)
- Blocks binding of its ligand, stromal cell-derived factor-1 alpha (SDF-1alpha/CXCL12)
- Disrupts the bond holding hematopoietic stem cells in the bone marrow niche, resulting in mobilization of CD34+ cells into the peripheral circulation
Contraindications
- Hypersensitivity to plerixafor or any of its components
Adverse Reactions
- Diarrhea
- Nausea
- Vomiting
- Injection site reactions (erythema, bruising, pruritus, pain)
- Fatigue
- Headache
- Arthralgia
- Dizziness
- Anaphylactic reactions (rare)
Drug Interactions
- No formal clinical drug-drug interaction studies performed
- Low potential for pharmacokinetic drug interactions due to minimal CYP450 enzyme inhibition or induction and low plasma protein binding
- Potential additive effects with other agents that mobilize hematopoietic stem cells (e.g., G-CSF)
