imlifidase
L04AA41
Forms & Strengths
- Intravenous solution: 11 mg/1.2 mL single-dose vial
Adult Dosing
- 0.25 mg/kg administered as a single intravenous infusion over 15 minutes prior to kidney transplantation
- Pre-medication with antihistamines and corticosteroids recommended to reduce infusion-related reactions
Pediatric Dosing
- Safety and efficacy in pediatric patients below the age of 18 years have not been established
Indications
- Desensitization treatment of highly sensitized adult patients awaiting kidney transplantation with a positive crossmatch against an available deceased donor
Mechanism of Action
- Cysteine endopeptidase derived from Streptococcus pyogenes that specifically cleaves human IgG, inactivating it and preventing IgG-mediated immune responses such as complement activation and antibody-mediated rejection
Contraindications
- Active systemic infection
- Hypersensitivity to imlifidase or its excipients
Adverse Reactions
- Infusion-related reactions (flushing, hypotension, urticaria, dyspnea)
- Severe infections
- Infusion site reactions
- Headache
- Nausea
Drug Interactions
- Immunosuppressants (co-administered for transplantation maintenance, but may have altered efficacy during imlifidase-induced IgG cleavage window)
- Endogenous and therapeutic IgG molecules (rapidly cleaved and inactivated by imlifidase; avoid administration of intravenous immunoglobulin [IVIG] immediately before or during imlifidase treatment)
