Forms & Strengths
- Tablet 500 mg
- Topical cream/gel 5%
Adult Dosing
- Oral: 500 mg to 1 g daily divided into 2 to 3 doses
- Topical: Apply to affected area 2 to 3 times daily
Pediatric Dosing
- Safety and efficacy not established in pediatric patients
Indications
- Management of rheumatoid arthritis
- Osteoarthritis
- Musculoskeletal pain and inflammation
Mechanism of Action
- Inhibition of cyclooxygenase (COX-1 and COX-2), leading to reduced synthesis of pro-inflammatory prostaglandins
Contraindications
- Hypersensitivity to alclofenac or other NSAIDs
- Active peptic ulcer disease or gastrointestinal bleeding
- Severe hepatic or renal impairment
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Adverse Reactions
- Maculopapular skin rashes and allergic dermatitis (high frequency leading to withdrawal)
- Gastrointestinal distress, nausea, dyspepsia, and peptic ulceration
- Elevated liver enzymes
- Dizziness, headache
Drug Interactions
- Anticoagulants (increased risk of bleeding)
- Antihypertensive agents and diuretics (decreased efficacy)
- Lithium and methotrexate (decreased renal clearance and increased toxicity)
- Other NSAIDs or corticosteroids (increased risk of gastrointestinal adverse effects)
