Forms & Strengths
- Oral capsules 300 mg
- Suppositories 300 mg
- Topical cream 3%
Adult Dosing
- Oral: 300 mg 1 to 2 times daily
- Rectal: 300 mg daily via suppository
- Topical: Apply to affected area 2 to 3 times daily
Pediatric Dosing
- Safety and efficacy not established in children under 18 years of age
Indications
- Management of pain and inflammation in osteoarthritis
- Treatment of rheumatoid arthritis
- Relief of acute musculoskeletal disorders and soft tissue injuries
Mechanism of Action
- Non-steroidal anti-inflammatory drug (NSAID)
- Inhibits cyclooxygenase (COX-1 and COX-2) enzymes
- Reduces synthesis of pro-inflammatory prostaglandins
Contraindications
- Hypersensitivity to difenpiramide or other NSAIDs
- Active peptic ulcer disease or gastrointestinal bleeding
- Severe hepatic or renal impairment
- Third trimester of pregnancy
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Adverse Reactions
- Epigastric pain, nausea, dyspepsia, and heartburn
- Peptic ulceration or gastrointestinal bleeding
- Dizziness, headache, and drowsiness
- Skin rash and pruritus
- Fluid retention and edema
Drug Interactions
- Increased risk of gastrointestinal bleeding with concurrent anticoagulants (e.g., warfarin)
- Enhanced nephrotoxicity and reduced efficacy with diuretics and ACE inhibitors
- Increased plasma concentrations and toxicity of lithium and methotrexate
- Additive ulcerogenic effects with corticosteroids and other NSAIDs
