Adult Dosing
- 150 mg orally 2 to 3 times daily after meals
- Maximum daily dose: 450 mg
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- Management of rheumatoid arthritis
- Osteoarthritis
- Ankylosing spondylitis
- Painful inflammatory conditions of the musculoskeletal system
Mechanism of Action
- Non-steroidal anti-inflammatory drug (NSAID)
- Inhibits cyclooxygenase (COX-1 and COX-2) enzymes, reducing prostaglandin synthesis
Contraindications
- Hypersensitivity to proglumetacin, indomethacin, or other NSAIDs
- Active peptic ulcer disease or history of recurrent gastrointestinal ulceration/bleeding
- Severe heart failure
- Severe hepatic impairment
- Severe renal impairment
- Third trimester of pregnancy
Adverse Reactions
- Epigastric pain
- Nausea and vomiting
- Diarrhea or constipation
- Gastrointestinal bleeding
- Headache
- Dizziness
- Drowsiness
- Skin rash
- Elevated liver enzymes
- Fluid retention and edema
Drug Interactions
- Anticoagulants (e.g., warfarin) increase risk of bleeding
- Antiplatelet agents and SSRIs increase gastrointestinal bleeding risk
- Other NSAIDs or corticosteroids increase risk of GI adverse events
- Antihypertensives and diuretics may have decreased efficacy due to prostaglandin inhibition
- Lithium and methotrexate clearance may be reduced, increasing toxicity risk
