Forms & Strengths
- Capsules: 100mg, 200mg, 400mg, 600mg
- Tablets: 200mg, 400mg, 600mg
- Oral suspension: 20mg/mL
Adult Dosing
- Osteoarthritis: 400mg tid
- Rheumatoid arthritis: 400mg tid or 600mg bid
- Juvenile rheumatoid arthritis: 20mg/kg/day in 3-4 divided doses
Pediatric Dosing
- Juvenile rheumatoid arthritis: 20mg/kg/day in 3-4 divided doses, max 30mg/kg/day
- Children over 2 years: 15-30mg/kg/day in 3-4 divided doses
Indications
- Relief of signs and symptoms of osteoarthritis
- Relief of signs and symptoms of rheumatoid arthritis
- Relief of signs and symptoms of juvenile rheumatoid arthritis
Mechanism of Action
- Inhibition of prostaglandin synthesis by competitively blocking the enzyme cyclooxygenase (COX)
- Reduces inflammation, pain, and fever
Contraindications
- Hypersensitivity to tolmetin or other NSAIDs
- Active peptic ulcer disease
- Severe renal impairment
- History of asthma or allergic reactions after taking aspirin or other NSAIDs
Adverse Reactions
- Gastrointestinal: nausea, vomiting, diarrhea, abdominal pain
- Central nervous system: headache, dizziness, drowsiness
- Renal: increased serum creatinine, renal insufficiency
- Hepatic: elevated liver enzymes
- Dermatologic: rash, pruritus
Drug Interactions
- Increased risk of bleeding with anticoagulants, antiplatelet agents, and SSRIs
- Increased risk of nephrotoxicity with diuretics, ACE inhibitors, and cyclosporine
- Decreased effectiveness of diuretics and antihypertensives
- Increased risk of hyperkalemia with potassium-sparing diuretics
