- Tablets: 250mg, 500mg
- Capsules: 250mg, 500mg
- Oral suspension: 250mg/5mL
Adult Dosing
- 250-500mg PO TID-QID
- Max dose: 750-1000mg PO QID
- Duration: up to 2-3 weeks
Pediatric Dosing
- Safety and efficacy not established in children under 12 years
- 12 years and older: 250-500mg PO TID-QID

- Muscle spasms
- Acute musculoskeletal disorders
- Lower back pain
- Cervical spine disorders
- Musculoskeletal injuries
Mechanism of Action
- Centrally acting muscle relaxant
- Inhibits multisynaptic reflex arcs
- Blocks nerve impulses
- Reduces muscle tone
Contraindications
- Hypersensitivity to chlorzoxazone
- Severe liver disease

Drug Interactions
- Alcohol: increased CNS depression
