pethidine, combinations excl. psycholeptics
N02AB52
Forms & Strengths
- Oral tablet formulations containing pethidine hydrochloride combined with non-opioid analgesics or antispasmodics (varies by regional availability)
- Parenteral combination solutions for injection (historically available in select markets)
Adult Dosing
- Acute moderate to severe pain: Oral/Parenteral doses equivalent to 50-150 mg pethidine hydrochloride every 3-4 hours as needed
- Max daily dose: Typically not to exceed 600 mg/24 hours due to risk of toxic metabolite accumulation (normeperidine)
Pediatric Dosing
- Safety and efficacy in pediatric patients not well established for combination products
- Generally avoided in children due to high risk of central nervous system toxicity and respiratory depression
Indications
- Short-term management of acute moderate to severe pain where combination therapy is deemed appropriate
- Relief of acute visceral spasm or biliary colic (in specific regional formulations)
Mechanism of Action
- Pethidine acts primarily as a mu-opioid receptor agonist providing central analgesia
- May possess local anesthetic properties and anticholinergic-like effects depending on the specific combination agent
Contraindications
- Hypersensitivity to pethidine or any components of the combination product
- Concomitant MAO inhibitor use or within 14 days of cessation (risk of severe serotonin syndrome)
- Significant respiratory depression, acute or severe bronchial asthma, or hypercarbia
- Known or suspected gastrointestinal obstruction, including paralytic ileus
Adverse Reactions
- Central nervous system: Dizziness, sedation, confusion, dysphoria, seizures (due to normeperidine accumulation)
- Respiratory: Respiratory depression
- Cardiovascular: Hypotension, tachycardia, syncope
- Gastrointestinal: Nausea, vomiting, constipation, dry mouth
- Other: Urinary retention, diaphoresis, tolerance and dependence with chronic use
Drug Interactions
- Monoamine Oxidase Inhibitors (MAOIs): Absolute contraindication due to risk of fatal hyperpyrexia, excitation, or coma
- CNS depressants (alcohol, benzodiazepines, general anesthetics): Potentiation of central nervous system and respiratory depression
- Serotonergic drugs (SSRIs, SNRIs, tricyclics): Increased risk of serotonin syndrome
- CYP3A4 inducers/inhibitors: May alter the metabolism of pethidine and its active metabolite
