Forms & Strengths
- Tablets: 5 mg
- Solution for injection: 5 mg/mL
Adult Dosing
- Oral: 2.5 mg to 5 mg every 4 to 6 hours as needed
- Subcutaneous or Intramuscular: 5 mg every 4 to 6 hours as needed
Pediatric Dosing
- Safety and efficacy not established in pediatric patients; use generally avoided
Indications
- Management of severe acute and chronic pain unresponsive to weaker analgesics
Mechanism of Action
- Full mu-opioid receptor agonist in the central nervous system, inhibiting ascending pain pathways and altering perception of and response to pain
Contraindications
- Hypersensitivity to dextromoramide
- Significant respiratory depression
- Acute or severe bronchial asthma
- Known or suspected paralytic ileus
- Concurrent use with monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuation
Adverse Reactions
- Respiratory depression
- Somnolence
- Dizziness
- Constipation
- Nausea and vomiting
- Hypotension
- Physical dependence and tolerance
Drug Interactions
- Central nervous system depressants (alcohol, benzodiazepines, other opioids) increase risk of profound sedation, respiratory depression, coma, and death
- Serotonergic drugs (SSRIs, SNRIs, MAOIs) increase risk of serotonin syndrome
- Potent CYP3A4 inhibitors may increase plasma concentrations of dextromoramide
