piritramide
N02AC03
Forms & Strengths
- Solution for injection: 10 mg/mL
Adult Dosing
- Subcutaneous or intramuscular injection: 15 mg (range 7.5-30 mg) as needed
- Slow intravenous injection: 7.5-15 mg titrated to pain relief
- Maximum daily dose and dosing interval must be individualized based on clinical response and tolerance
Pediatric Dosing
- Safety and efficacy in pediatric patients are not well established; use generally not recommended or restricted to specialized pediatric anesthesia settings
Indications
- Management of severe acute and chronic pain
- Premedication and postoperative pain management
Mechanism of Action
- Potent full mu-opioid receptor agonist in the central nervous system
- Inhibits ascending pain pathways, alters perception of and response to pain, and produces CNS depression
Contraindications
- Hypersensitivity to piritramide
- Severe respiratory depression or acute respiratory insufficiency
- Acute bronchial asthma
- Paralytic ileus
- Coma
Adverse Reactions
- Respiratory depression
- Sedation, somnolence, dizziness
- Nausea and vomiting
- Constipation
- Urinary retention
- Hypotension, bradycardia
- Pruritus, sweating
Drug Interactions
- CNS depressants (alcohol, benzodiazepines, barbiturates, general anesthetics): profound sedation, respiratory depression, coma, and death
- MAO inhibitors: potential for severe potentiation leading to CNS excitation or depression
- Mixed agonist-antagonist opioids (e.g., buprenorphine, pentazocine): may precipitate withdrawal symptoms or reduce analgesic efficacy
