pheneturide
N03AX13
Adult Dosing
- Initial: 200 to 400 mg daily in divided doses
- Maintenance: Titrate gradually up to 600 to 1200 mg daily based on clinical response and tolerability
- Maximum recommended daily dose: 1600 mg
Pediatric Dosing
- Safety and efficacy in pediatric patients under 6 years are not well established
- Children >6 years: Initial 100 to 200 mg daily, titrated gradually based on weight and clinical response
Indications
- Management of refractory generalized tonic-clonic seizures
- Treatment of complex partial seizures resistant to first-line antiepileptic drugs
Mechanism of Action
- Derivatives of phenylacetylurea sharing structural and pharmacological similarities with hydantoins and barbiturates
- Stabilizes neuronal membranes and suppresses seizure spread by prolonging the inactive state of voltage-gated sodium channels
Contraindications
- Hypersensitivity to pheneturide, other acetylurea derivatives, or hydantoins
- Severe hepatic impairment or active liver disease
- Severe renal impairment
- History of drug-induced bone marrow depression or severe hematologic disorders
Adverse Reactions
- Central nervous system: Drowsiness, dizziness, ataxia, fatigue, headache
- Gastrointestinal: Nausea, vomiting, anorexia, abdominal pain
- Hematologic: Leukopenia, thrombocytopenia, aplastic anemia (rare but severe)
- Hepatic: Elevated transaminases, drug-induced hepatitis
- Dermatologic: Rash, pruritus, Stevens-Johnson syndrome (rare)
Drug Interactions
- CYP enzyme inducers (e.g., carbamazepine, phenobarbital): May decrease pheneturide plasma concentrations
- CYP enzyme inhibitors: May increase pheneturide toxicity
- Central nervous system depressants (e.g., alcohol, opioids, benzodiazepines): Enhanced sedative and respiratory depressant effects
- Coumarin anticoagulants: Anticoagulant effect may be altered due to hepatic enzyme induction or displacement
Curated Content: Needs Vetting Before Put to Clinical Use
