Forms & Strengths
- Fluphenazine hydrochloride 1mg/mL oral solution
- Fluphenazine hydrochloride 2.5mg, 5mg, 10mg tablets
- Fluphenazine decanoate 25mg/mL long-acting injection
Adult Dosing
- Oral: 2.5-10mg daily in divided doses
- IM injection: 1.25-2.5mg every 3-4 days
- Long-acting injection: 12.5-25mg every 1-4 weeks
Pediatric Dosing
- Oral: 0.25-0.5mg daily in divided doses for children 6-12 years
- IM injection: not recommended for children under 12 years
- Long-acting injection: not recommended for children under 12 years
Indications
- Schizophrenia
- Acute and chronic psychoses
- Severe anxiety
- Agitation and restlessness
Mechanism of Action
- Blocks dopamine D2 receptors in the brain
- Antipsychotic and antiemetic effects
- Inhibits the dopamine-mediated transmission of nerve impulses
Contraindications
- Hypersensitivity to fluphenazine or other phenothiazines
- Coma or severe CNS depression
- Bone marrow depression
- Pregnancy and lactation
Adverse Reactions
- Extrapyramidal symptoms (e.g. dystonia, parkinsonism)
- Sedation and drowsiness
- Dry mouth and blurred vision
- Hypotension and orthostatic hypotension
- Neuroleptic malignant syndrome
Drug Interactions
- Increased risk of extrapyramidal symptoms with other antipsychotics
- Enhanced CNS depression with alcohol, barbiturates, and benzodiazepines
- Hypotensive effects increased with other antihypertensive agents
- Reduced efficacy of fluphenazine with concomitant use of antacids
