Forms & Strengths
- Tablets: 20mg
- Oral solution: not available
Adult Dosing
- Initial: 20mg orally once weekly
- Maintenance: 20-40mg orally once weekly
Pediatric Dosing
- Not recommended for patients under 12 years old
- For patients 12-18 years old, use with caution, no established dose
Indications
- Treatment of chronic schizophrenia
- Treatment of acute schizophrenia if oral medication is not feasible
Mechanism of Action
- Long-acting oral antipsychotic with dopamine D2 receptor antagonist activity
- Blocks dopamine receptors in the brain, reducing psychotic symptoms
Contraindications
- Hypersensitivity to penfluridol or other antipsychotics
- Severe CNS depression, comatose state, or bone marrow suppression
Adverse Reactions
- Extrapyramidal symptoms (dystonia, parkinsonism, akathisia)
- Sedation, dry mouth, blurred vision, constipation, orthostatic hypotension
- Increased risk of stroke and transient ischemic attack in elderly patients with dementia
Drug Interactions
- Concomitant use with CNS depressants may increase sedative effects
- Interaction with antihypertensive agents, may increase risk of hypotension
- Caution with concomitant use of serotonin reuptake inhibitors, may increase risk of serotonin syndrome
