Forms & Strengths
- Oral powder 1.5g

Adult Dosing
- Cognitive impairment: 1,000 mg to 1,500 mg orally once to twice daily
- Acute cerebrovascular disorders: 1,500 mg orally daily in divided doses
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Indications
- Cognitive decline and memory impairment in the elderly
- Cerebrovascular insufficiency
- Post-stroke cognitive deficits (off-label)
Mechanism of Action
- Positively allosterically modulates AMPA glutamate receptors in the brain
- Increases release and prevents desensitization of glutamate
- Enhances cholinergic neurotransmission and increases dopamine and serotonin turnover in the prefrontal cortex
Contraindications
- Hypersensitivity to aniracetam or other pyrrolidone derivatives
- Severe renal impairment (CrCl < 30 mL/min)
- Severe hepatic impairment
- Huntington's chorea (may exacerbate symptoms)
Adverse Reactions
- Nausea and gastrointestinal distress
- Headache
- Anxiety and nervousness
- Insomnia and sleep disturbances
- Vertigo
Drug Interactions
- Potential additive stimulant effects with central nervous system stimulants
- May interact synergistically with cholinesterase inhibitors (e.g., donepezil)
- Metabolized by CYP2C9 and CYP2D6; caution with inhibitors or inducers of these enzymes
