Forms & Strengths
- Intraocular solution: 0.01% (100 mcg/mL)
Adult Dosing
- Intraocular: 0.5 mL instilled into the anterior chamber of the eye during surgery to produce miosis
Pediatric Dosing
- Safety and efficacy not established in pediatric patients
Indications
- Miosis induction during intraocular surgery (e.g., cataract extraction, keratoplasty, iridectomy)
Mechanism of Action
- Direct parasympathomimetic agent that stimulates muscarinic receptors on the iris sphincter muscle, causing pupillary constriction (miosis), and on the ciliary muscle, promoting contraction and outflow facility
Contraindications
- Hypersensitivity to carbachol or any component of the formulation
- Acute inflammatory disease of the anterior chamber
Adverse Reactions
- Corneal clouding
- Persistent miosis
- Transient ocular elevation (intraocular pressure)
- Iritis
- Allergic reactions
- Corneal edema
Drug Interactions
- Topical ophthalmic anticholinergics or mydriatics (e.g., atropine) antagonize the miotic effect of carbachol
- Concurrent use with other cholinergic agonists may result in additive systemic or local adverse effects
