clofenotane, combinations
P03AB51
Forms & Strengths
- Topical lotion: clofenotane (DDT) combined with active ingredients like benzyl benzoate and benzocaine
- Topical liquid/spray formulations for ectoparasite eradication
Adult Dosing
- Apply topically to affected skin/hair areas as directed by specific combination product labeling
- Single application typically left for a specified duration (e.g., 24 hours) followed by thorough washing
- Repeat application generally discouraged unless clinically indicated after 7-10 days
Pediatric Dosing
- Safety and efficacy in neonates and infants not well established; avoid use in very young infants due to systemic absorption risks
- For older children, apply sparingly to minimize percutaneous absorption and neurotoxicity risk
Indications
- Eradication of pediculosis capitis (head lice)
- Treatment of scabies and other ectoparasite infestations where resistance has not rendered it obsolete
Mechanism of Action
- Clofenotane (DDT) acts on the nerve cell membranes of arthropods, keeping sodium channels open and causing repeated nerve discharges, leading to paralysis and death
- Combination agents (e.g., benzyl benzoate, benzocaine) provide synergistic parasiticidal and local anesthetic effects
Contraindications
- Known hypersensitivity to clofenotane or any formulation excipients
- Application to inflamed, broken, or raw skin surfaces
- Use near eyes, mucous membranes, or urethral meatus
Adverse Reactions
- Local skin irritation, erythema, burning, or stinging at the application site
- Contact dermatitis and allergic skin reactions
- Potential central nervous system toxicity (tremors, dizziness) if absorbed systemically in large amounts
Drug Interactions
- Concurrent use of other topical neurotoxic insecticides should be avoided to prevent additive toxicity
- Lipophilic excipients or oils may enhance the percutaneous absorption of clofenotane, increasing systemic risk
