Forms & Strengths
- Nasal spray solution 0.05% (0.5 mg per spray)
Adult Dosing
- 1‑2 sprays per nostril every 4‑6 h as needed
- Maximum 2 sprays per nostril per dose
- Do not exceed 6 sprays (3 doses) in 24 h
Pediatric Dosing
- Not recommended <12 yr
- If used in 12‑18 yr: 1 spray per nostril every 6‑8 h, max 2 sprays per day
Indications
- Temporary relief of nasal congestion due to common cold, sinusitis, allergic rhinitis, or other upper‑respiratory irritants
Mechanism of Action
- Selective α1‑adrenergic receptor agonist causing vasoconstriction of nasal mucosal arterioles, reducing edema and airflow obstruction
Contraindications
- Severe hypertension or uncontrolled cardiovascular disease
- Narrow‑angle glaucoma
- Hyperthyroidism
- Concurrent MAOI therapy
- Pregnancy category C – avoid if possible
- Children <12 yr
Adverse Reactions
- Nasal burning, stinging or dryness
- Rebound congestion (rhinitis medicamentosa) with >3‑5 days continuous use
- Headache, dizziness
- Tachycardia or palpitations
- Transient hypertension
- Insomnia
Drug Interactions
- Other sympathomimetic decongestants – additive vasoconstriction
- MAO inhibitors – risk of hypertensive crisis
- β‑blockers or antihypertensives – may blunt hypotensive effects
- Tricyclic antidepressants – increased pressor response
- Cocaine – synergistic vasoconstriction
