hydrocortisone, combinations
R01AD60
Forms & Strengths
- Nasal spray: hydrocortisone aceponate 0.1% combined with intranasal decongestants or antihistamines
- Nasal drops: various fixed-dose combinations containing topical corticosteroid and vasoconstrictors
Adult Dosing
- 1-2 sprays or drops into each nostril 2-3 times daily as needed for acute rhinitis
- Limit continuous use to 3-5 days to prevent rhinitis medicamentosa from decongestant components
Pediatric Dosing
- Safety and efficacy in children under 6 years generally not established for combination products
- Use in older children restricted to lowest effective dose for shortest possible duration (max 3-5 days)
Indications
- Short-term symptomatic relief of severe nasal congestion
- Inflammatory nasal symptoms associated with acute rhinitis, allergic rhinitis, or rhinosinusitis
Mechanism of Action
- Hydrocortisone derivative provides topical anti-inflammatory, anti-allergic, and vasoconstrictive effects
- Combined sympathomimetic decongestant causes alpha-adrenergic receptor stimulation, leading to nasal mucosa vasoconstriction and reduced edema
Contraindications
- Hypersensitivity to hydrocortisone or any combination components
- Rhinitis sicca or atrophic rhinitis
- Narrow-angle glaucoma
- Recent nasal surgery or trauma where mucosal barrier is compromised
- Viral, fungal, or untreated bacterial nasal infections
Adverse Reactions
- Transient stinging, burning, or dryness of the nasal mucosa
- Rebound nasal congestion ( rhinitis medicamentosa) with prolonged use beyond 5 days
- Epistaxis
- Systemic corticosteroid or sympathomimetic effects if absorbed in large quantities (e.g., tachycardia, hypertension, nervousness)
Drug Interactions
- Monoamine oxidase inhibitors (MAOIs): concurrent use with sympathomimetic components may precipitate hypertensive crisis
- Tri-cyclic antidepressants or antihypertensives: potential potentiation of cardiovascular effects
- Strong CYP3A4 inhibitors (e.g., ritonavir): potential for increased systemic absorption and corticosteroid toxicity with chronic high-dose use
