flurbiprofen

Forms & Strengths
- Ophthalmic solution: 0.03%
- Oral lozenge / troche: 8.75 mg
Adult Dosing
- Ophthalmic: 1 drop instilled into the affected eye approximately every 30 minutes beginning 2 hours prior to surgery (total of 4 doses)
- Oral lozenge: 1 lozenge dissolved slowly in the mouth every 3 to 6 hours as needed for sore throat; maximum 5 lozenges per 24 hours
Pediatric Dosing
- Ophthalmic: Safety and efficacy not established in pediatric patients
- Oral lozenge: Not recommended for children under 12 years of age
Indications
- Inhibition of intraoperative miosis during cataract surgery
- Short-term relief of symptoms associated with acute sore throat
Mechanism of Action
- Nonsteroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase (COX-1 and COX-2) enzymes
- Reduces synthesis of prostaglandins, decreasing inflammation, pain, and miosis (ophthalmic)
Contraindications
- Hypersensitivity to flurbiprofen or any component of the formulation
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Active epithelial herpes simplex keratitis (ophthalmic use)
Adverse Reactions
- Transient burning and stinging upon instillation (ophthalmic)
- Ocular irritation, redness, and increased bleeding time of ocular tissues
- Oral irritation, tongue edema, or altered taste (lozenge)
- Systemic NSAID effects rare with topical/local administration but possible with excessive absorption
Drug Interactions
- Concomitant use with acetylcholine chloride or carbachol may cause antagonism
- Potential for additive adverse effects if concurrent systemic NSAIDs are used
- May decrease the efficacy of antihypertensive agents when absorbed systemically
