salbutamol and beclometasone
R03AK13
Forms & Strengths
- Inhalation aerosol, metered‑dose inhaler: 50 µg salbutamol + 100 µg beclometasone per actuation
- Inhalation aerosol, dry‑powder inhaler: 50 µg salbutamol + 100 µg beclometasone per dose
Adult Dosing
- 1–2 inhalations (50 µg/100 µg) twice daily for maintenance; may increase to 2 inhalations four times daily during exacerbations
Pediatric Dosing
- Children ≥12 yr: same as adult dosing
- Children 4–11 yr: 1 inhalation twice daily; may increase to 2 inhalations twice daily if needed
Indications
- Long‑term maintenance therapy of persistent asthma
- Adjunct therapy in COPD with an asthmatic component (where approved)
Mechanism of Action
- Salbutamol: selective β2‑adrenergic agonist → bronchodilation via smooth‑muscle relaxation
- Beclometasone: inhaled corticosteroid → genomic anti‑inflammatory effects, reduces airway hyper‑responsiveness
Contraindications
- Hypersensitivity to salbutamol, beclometasone, or formulation excipients
- Acute severe asthma attack requiring immediate bronchodilator (use short‑acting β2‑agonist alone)
- Primary adrenal insufficiency (systemic corticosteroid contraindicated)
Adverse Reactions
- β2‑agonist: tremor, palpitations, tachycardia, hypokalaemia, headache
- Corticosteroid: oropharyngeal candidiasis, dysphonia, cough, throat irritation, possible systemic effects at high dose (e.g., adrenal suppression)
Drug Interactions
- β‑blockers may blunt bronchodilator response
- Diuretics, glucocorticoids, or sympathomimetics may potentiate hypokalaemia and tachyarrhythmia
- CYP3A4 inhibitors (ketoconazole, ritonavir) may increase systemic exposure to beclometasone
- MAO inhibitors or tricyclic antidepressants may enhance sympathomimetic effects of salbutamol
