Forms & Strengths
- Inhalation aerosol: 750 mcg/metered dose
- Syrup: 2 mg/5 mL
- Tablet: 10 mg, 20 mg
- Combination formulations with expectorants or bronchodilators
Adult Dosing
- Inhalation: 1-2 puffs (750-1500 mcg) up to 4 times daily
- Oral tablet: 10 to 20 mg 3 to 4 times daily
- Syrup: 10 mL (4 mg) 3 to 4 times daily
Pediatric Dosing
- Children > 12 years: Same as adult dosing
- Children 6-12 years (Syrup/Tablet): 5-10 mg 3 to 4 times daily
- Inhalation (Children > 6 years): 1 puff as needed, max 4 times daily
Indications
- Bronchospasm associated with bronchial asthma
- Chronic obstructive pulmonary disease (COPD)
- Chronic bronchitis
Mechanism of Action
- Non-selective beta-2 adrenergic receptor agonist
- Relaxes bronchial smooth muscle
- Increases ciliary motility and inhibits mediator release from mast cells
Contraindications
- Known hypersensitivity to orciprenaline or sympathomimetics
- Tachyarrhythmias
- Hypertrophic obstructive cardiomyopathy (HOCM)
- First trimester of pregnancy (relative)
Adverse Reactions
- Tremor
- Palpitations and tachycardia
- Nervousness or anxiety
- Headache
- Muscle cramps
- Paradoxical bronchospasm
Drug Interactions
- Beta-blockers antagonize the bronchodilatory effect
- Other sympathomimetic agents increase toxicity risk
- Monoamine oxidase inhibitors (MAOIs) and tricyclic antidepressants potentiate vascular effects
- Potassium-wasting diuretics may worsen ECG changes with hypokalemia
