colfosceril palmitate
R07AA01
Forms & Strengths
- Intratracheal suspension: 108 mg/vial colfosceril palmitate (synthetic surfactant, component of Exosurf Neonatal)
- Reconstitution diluent: 8 mL sterile water for injection
Adult Dosing
- Not applicable (indicated exclusively for neonatal populations)
Pediatric Dosing
- Prophylaxis in premature infants at risk for respiratory distress syndrome (RDS): 5 mL/kg intratracheally as soon as possible after birth (ideally within 24 hours), followed by repeat doses at 12 and 24 hours if RDS develops
- Rescue treatment in infants with established RDS: 5 mL/kg intratracheally as soon as possible after diagnosis, with a repeat dose 12 hours later if RDS persists
Indications
- Prevention and treatment of respiratory distress syndrome (RDS) in premature infants
Mechanism of Action
- Synthetic pulmonary surfactant containing dipalmitoylphosphatidylcholine (DPPC), cetyl alcohol, and tyloxapol
- Lowers surface tension at the air-liquid interface in the alveoli, preventing alveolar collapse at end-expiration and reducing the work of breathing
Contraindications
- No specific absolute contraindications known in the setting of emergency neonatal RDS treatment when administered under experienced supervision
Adverse Reactions
- Transient bradycardia
- Oxygen desaturation
- Endotracheal tube blockage
- Pulmonary hemorrhage
- Apnea
- Mucus plugging
Drug Interactions
- No formal drug-drug interaction studies performed in neonates
- Concomitant medications typically include routine neonatal intensive care therapies (e.g., caffeine, parenteral nutrition, antibiotics); monitor closely for cardiorespiratory instability during administration
