Forms & Strengths
- Oral tablets 3 mg
- Solution for injection 0.5% (5 mg/mL)
Adult Dosing
- Oral: 3 to 6 mg three times daily after meals
- Intravenous/Intramuscular: 1 to 3 mg as a single dose, may be repeated as needed
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established; use is generally not recommended
Indications
- Respiratory stimulant
- Management of acute respiratory depression due to various causes (historically used in anesthesia recovery and drug overdose)
Mechanism of Action
- Central respiratory stimulant acting directly on the respiratory center in the medulla oblongata
- Acts as a peripheral chemoreceptor stimulant via the carotid and aortic bodies, increasing respiratory rate and depth
Contraindications
- Hypersensitivity to dimefline or related centrally acting stimulants
- Severe hypertension
- Epilepsy or history of convulsive disorders
- Mechanical airway obstruction
Adverse Reactions
- Central nervous system stimulation (anxiety, restlessness, insomnia, tremor)
- Convulsions in high doses
- Cardiovascular effects (tachycardia, palpitations, hypertension)
- Gastrointestinal effects (nausea, vomiting)
- Sweating and hypersalivation
Drug Interactions
- Monoamine oxidase inhibitors (MAOIs): May dangerously potentiate pressor effects and central nervous system stimulation
- Other central nervous system stimulants: Increased risk of additive toxicity and convulsions
- Analeptics: Concurrent use may precipitate seizures
