Forms & Strengths
- Ophthalmic solution 0.5% (5 mg/mL) in single‑dose unit containers
- Ophthalmic suspension 0.5% (if available)
Adult Dosing
- One drop (≈30 µL) to the affected eye(s) twice daily, beginning on the day of surgery
- Can be increased to every 8 h for severe inflammation
- Typical treatment duration 7–14 days
- Do not exceed 4 drops per eye per day
Pediatric Dosing
- Safety and efficacy not established in children <12 years; use adult regimen only if benefits outweigh risks
- Avoid in infants and young children; consider alternative therapy
Indications
- Post‑operative ocular inflammation and pain
- Anterior uveitis adjunct therapy
- Management of allergic conjunctivitis (off‑label)
Mechanism of Action
- Reversible, non‑selective inhibition of cyclooxygenase‑1 and ‑2
- Decreases synthesis of prostaglandins and thromboxanes in ocular tissues
- Reduces inflammatory cell migration and edema
Contraindications
- Known hypersensitivity to piroxicam or any NSAID
- Active ocular infection (bacterial, viral, fungal)
- Corneal ulcer or perforation
- History of NSAID‑induced asthma or urticaria
Adverse Reactions
- Transient burning, stinging, or itching upon instillation
- Conjunctival hyperemia
- Blurred vision
- Dry eye sensation
- Corneal epithelial toxicity or delayed healing
- Allergic contact dermatitis of peri‑ocular skin
Drug Interactions
- Concurrent systemic NSAIDs increase risk of systemic toxicity (renal, GI)
- Co‑administration with topical steroids may potentiate corneal melting
- Avoid simultaneous use of other topical NSAIDs to prevent additive toxicity
- CYP2C9 inhibitors (e.g., fluconazole) may raise systemic piroxicam levels, though minimal with ocular use
