Forms & Strengths
- Otic solution: 2% (as nitrate)
Adult Dosing
- Otic fungal infections: Instill 1 to 2 drops into the affected ear twice daily for up to 2 weeks
Pediatric Dosing
- Safety and efficacy in pediatric patients under 12 years have not been fully established; use adult dosing if prescribed by a specialist
Indications
- Treatment of fungal otitis externa (otomycosis)
Mechanism of Action
- Inhibits fungal cytochrome P450-dependent 14-alpha-demethylase, decreasing ergosterol synthesis and increasing fungal cell membrane permeability
Contraindications
- Hypersensitivity to miconazole or any component of the formulation
- Perforated tympanic membrane (if systemic absorption through the middle ear is a risk)
Adverse Reactions
- Local irritation
- Burning
- Pruritus
- Erythema at the application site
Drug Interactions
- Minimal systemic absorption via otic route; however, theoretical interactions with strong CYP3A4 inhibitors/inducers exist if significant absorption occurs
