secretin
V04CK01
Forms & Strengths
- Powder for reconstitution: 16 mcg per vial (synthetic human secretin)
Adult Dosing
- Pancreatic exocrine function test: 0.2 mcg/kg IV single injection over 1 minute
- Gastrinoma (Zollinger-Ellison syndrome) diagnosis: 0.2 mcg/kg IV single injection over 1 minute
Pediatric Dosing
- Pancreatic exocrine function test (neonates to adolescents): 0.2 mcg/kg IV single injection over 1 minute
- Gastrinoma diagnosis (neonates to adolescents): 0.2 mcg/kg IV single injection over 1 minute
Indications
- Stimulation of pancreatic secretions to aid in the diagnosis of pancreatic exocrine dysfunction
- Facilitation of secretin-stimulated pancreatic MRI (MRCP) visualization
- Diagnosis of gastrinoma (Zollinger-Ellison syndrome)
Mechanism of Action
- Binds to specific secretin receptors on pancreatic ductal cells, stimulating bicarbonate and water secretion
- Stimulates pepsin and gastrin release from G-cells in patients with gastrinomas, paradoxical to normal physiological suppression
Contraindications
- History of hypersensitivity to secretin or any of its excipients
- Acute pancreatitis (until inflammation has resolved)
Adverse Reactions
- Flushing
- Abdominal pain or cramping
- Nausea and vomiting
- Dizziness
- Injection site reactions
- Anaphylaxis (rare)
Drug Interactions
- Anticholinergic drugs may blunt the secretory response to secretin
- H2-receptor antagonists and proton pump inhibitors may alter baseline gastrin levels, confounding Zollinger-Ellison syndrome testing
- Alcohol may alter pancreatic secretory response
