VARIOUS › CONTRAST MEDIA › X-RAY CONTRAST MEDIA, IODINATED › Watersoluble, nephrotropic, high osmolar X-ray contrast media
metrizoic acid
V08AA02
Forms & Strengths
- Injectable solution (as sodium and/or meglumine salts), various concentrations (e.g., 60%, 75%)
Adult Dosing
- Varies by diagnostic procedure (e.g., excretion urography, angiocardiography, cerebral angiography)
- Dose range: 10 mL to 60 mL per administration, depending on the specific radiological indication and technique used
Pediatric Dosing
- Safety and efficacy in pediatric patients are not fully established; use lowest effective dose if deemed necessary by specialist
- Dosing must be individualized based on age, body weight, and clinical condition
Indications
- Diagnostic radiocontrast agent for excretory urography
- Angiocardiography
- Peripheral and cerebral arteriography
- Venography
Mechanism of Action
- Contains organically bound iodine that absorbs X-rays, thereby enhancing visualization of blood vessels, urinary tract, and other internal structures during radiographic imaging
Contraindications
- Known hypersensitivity to metrizoic acid or other iodinated contrast media
- Manifest hyperthyroidism
- Severe dehydration
Adverse Reactions
- Mild to moderate anaphylactoid reactions (urticaria, nausea, vomiting, flushing)
- Injection site reactions (pain, warmth)
- Rare severe adverse events: severe anaphylaxis, bronchospasm, hypotension, renal impairment, cardiac arrhythmias
Drug Interactions
- Interferes with thyroid uptake of radioactive iodine
- Increased risk of renal toxicity when administered concurrently with nephrotoxic drugs (e.g., aminoglycosides, NSAIDs)
- Biguanides (metformin) may accumulate and induce lactic acidosis if acute renal impairment occurs post-contrast administration
