Forms & Strengths
- Injection: 51% (242 mg iodine/mL)
- Injection: 68% (320 mg iodine/mL)
- Injection: 74% (350 mg iodine/mL)
Adult Dosing
- Intravenous urography: 50 to 100 mL of 68% or 74% solution
- Computed tomography (CT) head/body: 50 to 200 mL depending on concentration and scan type
- Intravascular angiography: Individualized based on vessel size and procedure (typically 5 to 100 mL)
Pediatric Dosing
- Intravenous urography: 1 to 2 mL/kg of 68% solution (max 3 mL/kg)
- CT imaging: 1 to 2 mL/kg of 51% or 68% solution
Indications
- Diagnostic contrast agent for intravascular angiography
- Contrast-enhanced computed tomography (CT) head and body imaging
- Excretory urography
Mechanism of Action
- Low-osmolar, non-ionic, water-soluble iodinated radiographic contrast agent
- Tri-iodinated benzene ring structure minimizes systemic toxicity and osmolality compared to ionic agents
- Absorbs X-radiation, providing high-contrast visualization of blood vessels and anatomical structures during imaging
Contraindications
- Known hypersensitivity to ioversol or other iodinated contrast media
- Manifest thyrotoxicosis
Adverse Reactions
- Transient heat sensation, pain, or flushing at injection site
- Nausea, vomiting, and headache
- Hypersensitivity reactions ranging from mild urticaria to severe anaphylaxis
- Contrast-induced nephropathy (particularly in patients with preexisting renal impairment)
- Extravasation leading to local tissue irritation and swelling
Drug Interactions
- Metformin: Risk of lactic acidosis due to acute contrast-induced renal dysfunction; withhold metformin at time of or prior to procedure
- Nephrotoxic drugs (e.g., aminoglycosides, NSAIDs): Increased risk of renal impairment
- Beta-blockers: May exacerbate severe hypersensitivity/anaphylactoid reactions and reduce responsiveness to epinephrine rescue
