iodixanol
V08AB09
Forms & Strengths
- Injectable solution 270 mg iodine/mL
- Injectable solution 320 mg iodine/mL
Adult Dosing
- Coronary angiography/ventriculography: 50-80 mL (Visipaque 320)
- Computed tomography (CT) head/body: 50-150 mL depending on concentration
- Intravenous pyelography (IVP): 40-100 mL
Pediatric Dosing
- Angiocardiography (neonates and infants): 1-2 mL/kg (Visipaque 320)
- CT head/body (pediatric): 1-2 mL/kg of 270 mg/mL or 320 mg/mL solution
Indications
- Diagnostic radiopaque contrast agent for intra-arterial and intravenous procedures
- Cardiovascular imaging including coronary angiography and ventriculography
- Contrast-enhanced computed tomography (CECT) of head and body
- Excretory urography
Mechanism of Action
- Iodine atoms in the molecular structure absorb X-radiation, enhancing visualization of blood vessels and internal organs
- Iso-osmolar, non-ionic dimeric contrast medium designed to minimize hemodynamic and osmotic disturbances
Contraindications
- Severe hypersensitivity to iodixanol or other iodinated contrast media
- Untreated manifest hyperthyroidism
Adverse Reactions
- Transient warmth, flushing, or pain at injection site
- Nausea and vomiting
- Contrast-induced acute kidney injury (CI-AKI)
- Hypersensitivity reactions ranging from mild rash to severe anaphylaxis
Drug Interactions
- Biguanides (metformin): Risk of lactic acidosis if renal function acutely declines; withhold metformin
- Nephrotoxic medications (e.g., NSAIDs, aminoglycosides): Increased risk of renal impairment
- Interleukin-2: Increased risk of delayed hypersensitivity reactions
